## Find delays between abnormal results and provider action

Medrecords AI has no button that measures the delay between a result and the response to it. You trace it yourself with the timeline, search, filters and Q&A. For each abnormal result, you find the time it was reported, then the first note, call or order that responds. Every step links to the source page.

### Why this matters

Many med mal cases turn on a result that was reported but not acted on in time. A lab value comes back high. A scan shows a problem. The question is who saw it, and what they did next.

When you ask about a Medical Malpractice case, the assistant is told to look at whether results were acknowledged and acted on. It still does not measure delays for you, and it does not judge them. You build the list, and your expert decides what it means.

### Step 1: list the abnormal results

1. Open the **Timeline** tab. See [read the medical chronology](https://medrecords.ai/help/tasks/chronology/).
2. Press **Filters** and narrow by document type to lab results or imaging, if the list offers them.
3. Use search in **Keywords** mode. Try "critical", "abnormal", "high", "low" or "positive". Each word gets its own colour.
4. Click each hit to open the source page. Note the time the result was reported.

You can also open the **Q&A** pane and type in the box that says "Ask about this case": "Which lab or imaging results were abnormal, and when was each one reported?" Each answer cites its pages. Open every citation and check it.

### Step 2: find the first response

For each result, look for the first sign that someone saw it and acted.

- A nurse note that says the doctor was called or paged
- A new order, such as a repeat test or a new drug
- A progress note that names the result
- A transfer or a rapid response call

Search helps here. Put a phrase in quotes to find it as written, such as "notified Dr". Try "called", "paged" and "read back" too.

In Q&A, ask: "What is the first note after this result that mentions it or responds to it?" Check the cited page.

### Step 3: line them up

Step through the stay with **Date by date** on the timeline. Note each result and its first response in a simple table.

| Result | Time reported | First response | Time of response | Source pages |
| --- | --- | --- | --- | --- |
| Example: high potassium | 2:15 pm | Doctor paged, per nurse note | 6:40 pm | p. 41, p. 44 |

Keep the gap as 2 times from the records. Do not round them. If a time is missing, write "not stated" and ask for the record that would show it.

### Step 4: mark what you find

- Add a flag, such as "Result" or "Response", to each source page. You make your own flags under **Settings** , then **Stages and flags** .
- Add a note on the timeline entry with the time gap you found.
- Notes print beside the entry in the chronology PDF and exports.

See [add flags, highlights and notes](https://medrecords.ai/help/tasks/annotations/).

### Watch out for these traps

- **Times on fax headers.** A fax date can differ from the time a result was reported.
- **Undated notes.** Press **Undated Documents** on the timeline to see records with no date.
- **Late entries.** A note written later may describe an earlier time. Search for "late entry" and "addendum".
- **The audit trail.** The chart time and the time a note was signed can differ. Medrecords AI does not read EHR audit trails. Request the audit trail on its own. Our guide on [EHR audit trails](https://medrecords.ai/guides/ehr-audit-trail-medical-malpractice/) explains how.

### Turn it into a report

The Standard of Care Analysis has a section called Assessments and the Responses to Them. Check its facts against your own table. See [write the standard of care, causation and merit reports](https://medrecords.ai/help/case-types/medical-malpractice/reports/).

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
