## Work through a fictional med mal case: a missed lab result

This worked example follows a made-up med mal case in Medrecords AI. A lab result came back high after the patient went home, and no record shows a call to him. You will see how to set up the case, trace the result, find what is missing and draft the reports. All names, places and facts are invented.

### The fictional case

Walter is 58. On March 3, 2024, he goes to the emergency room at Harlow Creek Medical Center with chest pain. A blood test for heart damage is drawn at 1:50 pm. He is sent home at 3:30 pm. The lab reports the result as high at 4:20 pm.

On March 9, 2024, Walter comes back by ambulance. He is admitted with a heart attack. His lawyer, Grace, wants to know what happened to the March 3 result.

### Step 1: set up the case

Grace picks **Medical Malpractice** and uploads the emergency room chart, the lab reports and the March 9 hospital stay.

When the case shows **Needs confirming** , she checks the **Confirm case details** window.

- **Focus.** She keeps the item about the lab result and follow-up. She removes an old knee problem that is not part of the claim.
- **Key date.** She makes March 3, 2024 the main date. She presses **Add date** for March 9, 2024.
- **Forum state.** She checks the state where the matter will be filed.

She presses **Confirm and continue** . She will write the reports later in the Report studio. See [set up a medical malpractice case](https://medrecords.ai/help/case-types/medical-malpractice/setup/).

### Step 2: trace the result

On the **Timeline** tab, Grace picks **Date by date** and steps to March 3, 2024. She searches in **Keywords** mode for "troponin" and "high". Each hit opens the source page.

She builds a small table from the cited pages.

| Event | Time | Source |
| --- | --- | --- |
| Blood drawn | 1:50 pm | Lab report |
| Discharged home | 3:30 pm | Discharge note |
| Result reported as high | 4:20 pm | Lab report |
| First later note about the result | Not found | None |

Then she asks in Q&A: "Is there any note, call or message about the March 3 lab result after 4:20 pm?" The answer cites the pages it used. Grace opens each one. She finds no call note in the file.

She adds a flag called "Result" to the lab page and a note on the timeline entry: "Result after discharge. No call note found."

### Step 3: check what is missing

Grace opens the missing records list. She uses the **Type** menu and **Ordered** to look at lab items. She also searches for "callback", "phone" and "message".

No call log or message record is in the file. A missing call note may mean no call was made. It may also mean the records were not sent. Grace presses **Request from provider** to draft a request for the emergency room's call and message records. She also requests the EHR audit trail on its own, because Medrecords AI does not read audit trails. See [find records that are referenced but not produced](https://medrecords.ai/help/case-types/medical-malpractice/missing/).

### Step 4: read the reports

Grace opens the Report studio and makes the Standard of Care Analysis and the Causation Analysis. Each new report costs 100 credits, with 1 verification and cross-examination run included.

The Standard of Care Analysis lays out the care at issue, what was ordered, what was done, and the responses to each result. The Causation Analysis sets out the onset and the order of events from March 3 to March 9, 2024.

Grace presses **Run Verification and Cross-Examination** . She checks every claim marked Unsupported or Contradicted against the source page.

Neither report says whether the care fell below the standard, or whether the delay caused the heart attack. Those are questions for Walter's expert. Grace sends the expert the exported reports and her table.

### What this example shows

1. Confirm a tight focus and the right key dates first.
2. Trace each result to its first response, with times from the records.
3. Treat a missing note as a question to chase, not an answer.
4. Check every cited page before you rely on it.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
