- The NCCHC Standards for Health Services in Jails and in Prisons, which set receiving screening, sick call response and continuity of care requirements the facility either met or did not
- The facility’s own health services policies and its contract with the correctional health vendor, including the response times both commit to
- 42 U.S.C. §1997e(a), the Prison Litigation Reform Act exhaustion requirement, under which the grievance record has to be reconstructed alongside the medical one

- The federal district court at summary judgment on qualified immunity, which reads the medical file for what each defendant knew and when
- NCCHC accreditation surveyors and any monitor appointed under a consent decree, who audit the same records against the same standards
- The medical examiner in a death in custody, whose findings the case either adopts or has to overcome
