What is in this list

Every name here comes off a state regulator's own published roster of authorised self-insurers. Captive insurers were removed: a captive is a risk-financing vehicle, not an organisation that reads claim files. So were captive managers, auditors and vendors, which are suppliers to this market rather than part of it.

Coverage follows what states publish, which is uneven. California, Pennsylvania, Georgia, West Virginia and Louisiana publish large rosters; New York publishes guidance for self-insured employers but no downloadable list, so New York is thin here for a reason that has nothing to do with how many self-insurers it has.

Why record review reaches self-insurers

A self-insured employer carries its own claim costs, so the medical file is its own exposure rather than a carrier's. Some administer claims in-house, some hand them to a TPA, and the rosters that say which are rare. Either way somebody has to read the treatment history before a reserve is set or a settlement is signed.
