We condense and cite what the prior record already documents. We do not examine the claimant, author findings, offer a diagnosis or prognosis, or state whether any Listing is met. The examination and every clinical conclusion in the CE report belong to the examining physician.

In the demo case, page 140 turned out to belong to a different patient. The digest flags it and quarantines it in its own line, instead of folding it silently into the history the examiner reads.
