Can ChatGPT review medical records?

What chat tools do well, and where chart review needs more

General AI chat tools are fine for definitions and a few pages. For a 1,000-page chart you will sign an opinion on, you need a cite on every line, duplicates removed and a BAA in place.

Can ChatGPT review medical records for a standard-of-care review or an IME report?

It can explain terms and summarize a few pages, but it is the wrong tool for a 1,000-page chart you will sign an opinion on. General chat tools do not cite every line to its source page or remove duplicates across merged packets, and a standard consumer plan comes with no BAA.

General AI chat vs a record review tool, side by side

A chat tool is a good reference desk. A record review tool is built to read the whole chart, cite it and hand you a draft you can check. Here is how they compare on the tasks a physician reviewer, a risk manager or an IME examiner does.

Where a general chat tool is fine

Use it for the reference work that never touches a patient record:

  • Looking up a term, an abbreviation or a drug class.
  • Explaining a lab value or a scoring scale before you read the chart.
  • Summarizing a published guideline or a journal abstract.
  • Tightening the wording of a paragraph you wrote, with no identifiers in it.

The line is the record. Once the input is a patient chart, or the output feeds an opinion you will sign, the bar moves to citations, completeness and a BAA.

Why a page citation on every line matters

A standard-of-care opinion or an IME report rests on statements a reader can trace. If a summary says the patient was febrile on post-op day 2, you need the page that says so, and you need it fast. A chat answer that names no page, or names a page that says something else, sends you back to the full chart.

Medrecords AI cites every output line to its source page, and each cite opens that page. The point is not trust in the software. It is that checking a line takes 1 click instead of a search through 1,000 pages.

What happens to duplicates in a merged packet

Hospital productions repeat themselves: the same progress note arrives in the chart, the billing packet and a second production. A chat tool has no step that removes those repeats, so the same note can be summarized twice or counted as 2 events.

In a sample 342-page, 2-packet production, Medrecords AI deduplication found 11 duplicate pages and collapsed them to 1 canonical record. Billing counts unique pages only, and exact duplicates are never charged.

File size and context limits on a 1,000-page chart

Chat tools publish upload caps. OpenAI lists 512 MB per document and 2 million tokens per text or document file, with 3 uploads a day on the Free plan (OpenAI file uploads FAQ, read 3 Nov 2026). Other tools publish their own limits, and they change.

A cap tells you what the tool accepts, not what it read closely. On a long file, a chat answer may draw on the passages it found most relevant and not say which pages it skipped. For a chart you will base an opinion on, ask for the pages behind each answer and spot-check them, or use a tool that reads every page and cites each line. Medrecords AI reads about 100 pages a minute, so a 1,000-page chart takes about 10 minutes.

PHI, BAAs and which plan you are on

A patient chart is protected health information. Under HIPAA, a covered entity that shares PHI with a vendor needs a business associate agreement with that vendor. Here is what the 3 large AI vendors say in public, as read on 3 Nov 2026:

  • OpenAI. A BAA is available for the API and for sales-managed ChatGPT Enterprise and Edu accounts. OpenAI says it does not offer a BAA for ChatGPT Business, and it describes a separate in-product BAA flow for eligible clinicians on ChatGPT for Clinicians (OpenAI BAA article).
  • Anthropic. The BAA covers HIPAA-ready services such as the first-party API and Claude Enterprise, once the organization owner turns HIPAA on, and it excludes some features. Consumer plans such as Claude Free, Pro and Max are handled separately (Anthropic BAA article).
  • Google. Google Workspace customers can accept a BAA in the Admin console for the services on the HIPAA included functionality list, which includes some Gemini features in Workspace. Google says customers without a signed BAA must not use PHI in those services (Google Workspace HIPAA page).

OpenAI and Anthropic both sell health products now, and the plan decides the BAA answer. See Medrecords AI vs ChatGPT Health and Medrecords AI vs Claude for Healthcare for each product, price and file limit.

The pattern: BAAs exist on business, API and clinician plans, under configuration rules, and not on a standard consumer account. Vendors change these terms often. Check your plan, and ask your privacy officer, before any PHI goes into a chat window. Medrecords AI is SOC 2 and HIPAA compliant and signs a BAA at signup; see HIPAA at Medrecords AI.

Standard-of-care review with a chat tool

A standard-of-care review turns on what was known, when, and what was done next. That needs timed entries from several charts on 1 timeline, orders kept apart from results, and silences shown as rows. A chat summary written as prose tends to blur those times together.

Medrecords AI drafts that timeline with a cite on every entry and flags missing records with the page that implies them. The opinion on whether care met the standard stays with the reviewing physician. See medical malpractice record review for the workflow risk managers and physician reviewers use.

Drafting an IME report with a chat tool

An IME report lists every record reviewed, with the facility, the date span and the page range, then the history, the exam and the opinions. A chat tool can write fluent prose from what you paste. It cannot tell you which records it never saw, and the records-reviewed section is where a gap shows up first.

Medrecords AI drafts the report in your structure from the cited records. Causation, impairment and work status stay your opinions, and you sign. See IME record review for orthopedic and other IME work.

The workflow: Medrecords AI drafts and cites, you review and sign

  1. Upload the full production, every packet, in any order.
  2. Exact duplicates are collapsed and never charged.
  3. The draft comes back with every line cited to its source page and missing records flagged.
  4. You open the pages behind the lines your opinion depends on, edit, and sign.

The software gives no opinion of its own and signs nothing. A flag is a prompt for review, never a finding.

Sources and method

BAA terms and upload limits come from each vendor's public help pages, read on 3 Nov 2026. Vendors change these often, so check your plan before you rely on them.

Frequently asked questions

Can I paste a patient chart into ChatGPT?

Only if your organization is on a plan covered by a business associate agreement and you follow its configuration rules. OpenAI says it offers a BAA for sales-managed ChatGPT Enterprise and Edu accounts and for its API, and does not offer one for ChatGPT Business. If you are not sure which plan you are on, check your plan before any PHI goes in.

Does Claude or Gemini sign a BAA?

Anthropic says its BAA covers its first-party API and HIPAA-ready Claude Enterprise plans once the organization owner turns HIPAA on, and its consumer plans are handled separately. Google offers a BAA to Google Workspace customers for the services on its HIPAA included functionality list. A personal account on either is a different product, so check your plan.

Can ChatGPT write an IME report?

It can draft prose from what you paste in. It cannot show you which records it did not read, and it does not cite each statement to a source page by default. The records-reviewed section, the history and the opinions all need that trail, and the examiner signs the report.

Can a general chat tool cite the page a finding came from?

You can ask it to, and sometimes it will name a page. You still have to open that page and confirm it says what the answer says, line by line. A purpose-built tool such as Medrecords AI cites every output line to its source page, so the check is 1 click.

How large a file can ChatGPT take?

OpenAI lists a cap of 512 MB per document and 2 million tokens per text or document file, and Free users get 3 file uploads a day, as of 3 Nov 2026. A cap on what it accepts is not a promise that it read every page closely, so ask which pages an answer came from.

When is a general chat tool good enough?

For definitions, abbreviations, a drug class, or a summary of a few pages that hold no PHI or sit under a covered plan. Once the output feeds a standard-of-care opinion or a signed IME report, you need a cite on every line and a BAA in place.

What does Medrecords AI cost?

Pricing starts at 10 cents per deduplicated page, down to 5 cents with volume. You pay per page, not per case, exact duplicates are never charged, and there is no subscription or seat fee. First case is free on us.
