Life care plan cost-projection worksheet
A blank cost-projection worksheet for a life care plan: one row per recommended item, with its frequency, unit cost, start age, and duration, and formulas that carry each row to an annual and a lifetime figure. Totals recalculate as you edit, so a changed assumption updates the plan instead of forcing a rebuild.
What’s inside
- One row per recommended item, grouped by the standard cost categories
- Frequency, unit cost, start age, and duration as separate inputs you control
- Annual and lifetime columns computed by formula, never typed in
- A category subtotal block and a plan total that recalculate as you edit
- A source column, so every unit cost carries where the figure came from

What the file contains
A blank working file. It states no case data and no cost of its own. Built on Life care plan deliverable.
How the sheet is wired
| Blue cells | Yours to fill. Category, item, times per year, unit cost, start age, end age, source. |
| Years | Formula: end age minus start age, plus one. Leave both ages blank for a one-off item. |
| Annual cost | Formula: times per year multiplied by unit cost. |
| Lifetime cost | Formula: annual cost multiplied by years. |
| Source / basis | Where the unit cost came from. An uncited cost is the first one challenged. |
| Subtotals | SUMIF over the category column, so a row moves to a new subtotal when you retype its category. |
| What this does not do | It supplies no cost, no duration and no life expectancy. Those are your professional judgment. |
Categories come as a dropdown: medications, therapy and rehabilitation, physician and follow-up care, equipment and supplies, attendant and home care, diagnostics and imaging, surgical and procedural, home and vehicle modification, plus a free row for your own. Subtotals are SUMIF formulas over that column, so retyping a category moves the row to a different subtotal without touching anything else.

Common questions
Does the worksheet supply any costs?
No. It supplies structure and formulas. Every unit cost, duration and life expectancy is yours to research, enter and defend, which is the judgment you are retained for.
What does it calculate?
Years from the start and end ages, annual cost from frequency and unit cost, and lifetime cost from the two. Category subtotals and a plan total recalculate as you edit.
Can I change the categories?
Yes. They come as a dropdown built on the recurring-care categories we name on the life care plan page, and the subtotals are SUMIF formulas over that column, so your own category names work as soon as you type them.

The tool above is yours to run by hand. This is what the software does with the same file.
