How accurate is Medrecords AI, and what are its limits?
Medrecords AI ties every fact it gives you to a page in your records, so you can check it in 1 click. It also checks its own answers and reports against those pages. It can still misread a poor scan or handwriting. It makes no clinical or legal judgments. Treat it as a cited first draft that you check.

Every fact has a source
- Chronology events. Each event is read from a page. Click the event and the page opens with the quoted sentence highlighted.
- Record details. The verbatim abstract shows the exact words from the record.
- Q&A answers. Each answer carries numbered citations. The cited text shows under the answer.
- Narrative. Each sentence carries a numbered citation.
- Reports. Claims cite the records they come from.

Checks the app runs for you
| Where | What is checked |
|---|---|
| Q&A answers | Each sentence is checked against the cited pages before you see it. False statements and made-up details, such as a wrong dose or date, are corrected |
| Reports | Run Verification and Cross-Examination checks each claim. Results show as Contradicted, Unsupported, Not checked and Supported. It also lists likely lines of attack, rated High, Medium and Low |
The first verification run comes with each full report. A later run costs 50 credits.
With Auto-refine report switched on, the run fixes unsupported claims by changing the report text. It is on by default. Turn it off in the report editor if you want to make every change yourself.
Where mistakes are most likely
- Handwriting. Doctors' notes and forms filled in by hand are the hardest to read.
- Poor scans. Faint, blurry or crooked pages give weaker text.
- Dates. A date on a fax header can differ from the visit date.
- Images. DICOM imaging files are stored and you can view them, but the app does not read them. They show "Not read".
We do not publish an accuracy percentage. Results depend on the quality of your records. Check the facts you plan to rely on.
What Medrecords AI does not do
- It does not make clinical or legal judgments. It states what the records say, with citations. Conclusions on negligence, causation or capacity are yours or your expert's.
- It does not vouch for the records. If a record is wrong, the summary of it will be wrong too.
- It does not fetch missing records. It lists records that other records mention. See the missing records list.
- It does not read TIFF, GIF or HEIC files. Save them as PDF or JPG first.
- It does not let you edit details after you confirm. Make fixes while the case says "Needs confirming".
How to work with it
- Confirm the case focus and key date carefully. See confirm the case details.
- Read the chronology around the key date first.
- Click through to the source for every fact you will quote.
- Run verification on reports before you send them.
