What this case type does
A nursing home file is long and repetitive. The same shift note can look almost the same for 90 days. The Nursing Home Neglect case type is set up for that kind of file.
- The key date is called Date of Admission. There is no accident date. Each hospital transfer and return starts a new stay.
- The confirm window asks for the Facility, the Admission period and the Incident type.
- When you ask questions, the assistant reads the chart by care area. These are skin and wounds, nutrition and weight, mobility and falls, continence, supervision and medication.
- It compares what the care plan promised with what the chart shows. Turning every 2 hours, weekly weights and skin checks are common examples.
- Similar shift notes on different days are kept as separate days. They are not treated as copies.
- The reports pre-selected for you are Record Review, Standard of Care Analysis and Damages Summary.
What it does not do
Be clear about the limits before you start.
- It does not conclude that neglect happened. That is for you and your expert.
- It does not cite F-tags or federal or state rules.
- It has no special pressure injury, fall or staffing tool. You build those timelines yourself with search, filters and the chronology.
- It does not pull staffing data (PBJ, the payroll-based staffing reports) or state inspection reports. It only reads what you upload.
The playbook, step by step
A good order to work in
- Collect the facility chart, hospital records and ambulance run sheets.
- Create the case and upload them.
- Confirm the case details with care. They lock after you confirm.
- Check the missing records list and look for gaps in the flowsheets.
- Build a timeline for each harm, such as a wound, a fall or weight loss.
- Write the report and run the verification check.
For the rules side of these cases, such as the MDS and the CMS-2567 inspection form, read our guide to nursing home records, the MDS and the CMS-2567.
Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
