Build pressure injury, fall and weight loss timelines
Medrecords AI has no special pressure injury, fall or staffing tool. You build each timeline yourself from the chronology, keyword search, filters and flags. Q&A answers with page citations help you check your work. The method is the same for every harm: find every mention, put it in date order, and compare it with the care plan.

The basic method
For any harm, work in 4 steps.
- Find every mention. Use keyword search on the Timeline tab.
- Flag what matters. Put a flag on each page you will use.
- Read it in order. Step through the chronology 1 date at a time.
- Compare with the care plan. Check what was promised against what the chart shows.
Before you start, make a few flags. Open Settings, then Stages and flags. Under Record flags, press New flag. Good names are "Skin", "Fall", "Weight" and "Care plan". See add flags, highlights and notes.
Pressure injury timeline
A pressure injury is a bed sore. Records often call it a pressure ulcer or a decubitus.
- Open the Timeline tab. In the search box, pick Keywords.
- Add these words 1 at a time: "pressure", "stage", "Braden", "wound", "reposition" and "turn". Each gets its own colour.
- Choose Any to see every record with at least 1 of them.
- Press Filters and pick the body part, such as the sacrum or heel, if the records name it.
- Under view mode, pick Date by date. Press Next date to step through.
- Flag the first note of red skin, each new stage, each Braden score and each wound measurement.
Then compare. Find the care plan's turning schedule, such as every 2 hours. Search the aide flowsheets for "turn" and "reposition" on the days before the wound got worse. Note any shift with no entry. That is a question for your expert, not a finding.
Fall timeline
- Search with Keywords for "fall", "floor", "found", "alarm" and "unwitnessed".
- Add the fall risk words your records use, such as "Morse" (a fall risk score).
- Flag each fall, each fall risk score and each post-fall note.
- Search the care plan for the fall steps it promised, such as a bed alarm or a low bed.
- Read the nurse notes for the 3 days after each fall. Check for head checks and a doctor or family call.
Weight loss and intake timeline
- Search with Keywords for "weight", "lbs", "meal", "intake", "supplement" and "dietitian".
- Write each weight and its date in a list as you go.
- Flag each dietitian note and each diet or supplement order.
- Compare the care plan's weight schedule, such as weekly weights, with the weights on file.
Entries made while the resident was away
Facility notes dated during a hospital stay are worth a close look. The app does not flag these for you. Do it by hand.
- Find each hospital stay in the chronology. Note the dates out and back.
- Open the filters. Set Event Date From and Event Date To to those dates.
- Set Group By to facility. Look for facility entries inside the hospital dates.
- Open the source page of each 1. See check any fact against its source page.
Ask questions with citations
Q&A answers cite the pages they come from. On this case type, the assistant reads the chart by care area. It compares what the care plan promised with what the chart shows. Open the Q&A pane and type in the box that says "Ask about this case". Some questions to try:
- "List every Braden score with its date."
- "What turning schedule does the care plan set, and when was it changed?"
- "List every weight recorded from admission to discharge."
- "When was the doctor or family told about the wound?"
Click each citation and read the page. The assistant does not say whether care fell below the standard. That call is yours and your expert's.
Keep notes as you go
Press the note button on an entry to add a remark. Notes print beside the entry in the chronology PDF and in exports. Use them to record each gap you find.
When the timelines are done, see choose a report for this case. For a walk-through, see the fictional pressure injury example. Our guide on pressure injury case records covers which records show wound care.
Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
