Set up a nursing home case: facility, admission and incident

To set up a nursing home case in Medrecords AI, pick the Nursing Home Neglect case type and upload the records. After the first read, the app asks you to confirm the facility, the admission period and the incident type. It fills these in from the records where it can. You check them, fix them, and confirm.

Pick the case type

Pick Nursing Home Neglect when you create the case. A fall or a bed sore in a care home goes here, not under Personal Injury. This type brings the facility fields, the admission date and the nursing home reports.

Your role only changes the order of the case type list. Every role can pick this type. See choose the right case type.

When the details window opens

When the first read of the records is done, the case status becomes Needs confirming. The Confirm case details window opens on its own. If you closed it, press Confirm case on the banner at the top of the case.

The case details section

The window has a section called Case details. Nothing in it is required. A field you leave empty is saved as not stated in the records. An empty choice shows "Not stated in the records".

FieldWhat to put in it
Forum stateThe state where the matter is filed. Every case type has this field
FacilityThe name of the nursing home
Admission periodThe dates the resident lived there
Incident typeThe harm at the center of the case

The Incident type choices are:

  • Fall
  • Pressure ulcer
  • Medication
  • Dehydration / malnutrition
  • Elopement (the resident left the building without staff knowing)
  • Abuse
  • Infection
  • Other

Pick the 1 that matches the main harm. If the file has 2 harms, such as a fall and a wound, pick the main one. Then add the other to the focus list.

Set the Date of Admission

On this case type the key date is called "Date of admission". There is no accident date here.

  1. Check the date the app found. If it found more than 1, press Make primary on the right one.
  2. Press Add date for each hospital transfer and each return. Add a note to each one, such as "sent to hospital" or "back from hospital".
  3. Check the focus and body parts. Remove items that are not part of the claim. Use "+ add your own" for anything missing, such as a wound site.

The app treats each hospital trip and return as the start of a new stay. Adding those dates helps you read the chart in the right pieces.

Confirm

Press Confirm and continue. Confirming is free. It builds the chronology and the narrative.

You write reports later in the Report studio. A new report costs 100 credits and includes 1 verification and cross-examination run. See choose a report for this case.

Check every field first. After you confirm, you cannot edit the details, the focus, the key dates or the body parts. The 1 exception is when you add records later. The window then opens again and you can update your answers. See add records to an existing case.

What this setup does not do

The incident type tells the app what the case is about. It does not switch on a special wound, fall or staffing tool. Those do not exist yet. You build those timelines with search and the chronology. See build pressure injury, fall and weight loss timelines.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
