Track each claimed condition: diagnosis, event and nexus
Medrecords AI has no special tracker for each claimed condition. You build 1 yourself with Q&A, search, flags and the timeline. For each condition, find the current diagnosis, the event in service, the doctor's link between them, and the record of symptoms over time. Every answer cites the page it came from.

The 4 things to find
Work through 1 claimed condition at a time. For each one, look for these.
| Part | What you look for |
|---|---|
| Current diagnosis | A doctor names the condition today |
| In-service event | An injury, illness or exposure during service |
| Nexus | A doctor's opinion that links the condition to service. "Nexus" means that link |
| Continuity | Notes that show the symptoms carried on over the years |
The Q&A assistant on this case type reads the records against the first 3 parts and keeps them apart. A note that shows the diagnosis does not prove the event. You keep the same habit.
Step 1: ask the records
Open the Q&A pane in the case. Type your question in the box "Ask about this case". Each answer has numbered citations. The Ask a question button in the top bar is the help chat, not case Q&A. Ask about 1 part and 1 condition at a time. For example:
- "What is the most recent diagnosis of the left knee, and who made it?"
- "Is there any record of a knee injury during service?"
- "Which doctors give an opinion on whether the knee condition is related to service? Quote their words."
- "List every visit for knee pain after discharge, by date."
The assistant quotes a doctor's wording exactly. "At least as likely as not" and "possibly related" stay as written. It does not say whether service connection is met.
Step 2: search for the wording
Search finds every page, not only the ones Q&A picked. Use it to be sure you have them all.
- Use Keywords and put the phrase in quotes, such as "at least as likely as not" or "less likely than not".
- Add the condition as a second keyword and pick All.
- Use Exact for a code or a form number.
See search your records.
Step 3: flag what you find
Make 1 flag for each part, such as "Diagnosis", "In-service event", "Nexus" and "Continuity". Open Settings, then Stages and flags, and press New flag.
Then add a note to each key record and pick the flag. The note prints beside the record in the chronology PDF and in exports. Filter by Note flag to see every record for 1 part. See add flags, highlights and notes.
Step 4: read the timeline
- Open the Timeline tab.
- Search for the condition, such as "knee".
- Step through with Date by date. Look at the order: event in service, symptoms, diagnosis, opinion.
- Click any entry to see the source page with the quote highlighted.
See read the medical chronology.
Fit it to the theory
The theory you picked at setup changes what you look for.
- Secondary. Look for the first, connected condition and a doctor's link from it to the new one.
- Aggravation. Look for records from before service, and for notes on how the condition changed.
- Presumptive. Search for the exposure, such as the place or the substance named in the records.
- Direct. Look for the event in service and the link to it.
Step 5: write it up
When the parts are clear, write a report. The Causation Analysis sets out what the records say on the link, for and against. See which reports fit a VA claim.
