Find med mal records that are referenced but not produced
Medrecords AI lists records that your own documents mention but that are not in the file, such as a lab ordered with no result on file. In a med mal case, check that list first. Then use search and the timeline to look for gaps the list cannot see, because the app only finds what another record refers to.

Start with the missing records list
Open the list from the Needs attention card on the Overview, or from the missing records figure on the Case files tab. It costs no credits. The full steps are in work through the missing records list.
The list has 3 tabs:
| Tab | What it holds |
|---|---|
| Outstanding | Records still missing |
| Located | Records matched to a file in the case |
| Dismissed | Records you set aside, with a reason |
Work the list for a med mal case
- Use the Type menu to show lab, imaging and consult items first. These often hold the results your case turns on.
- Pick Ordered to see items that were ordered with no result on file.
- Open each provider group. Click through to the records that mention each item.
- Where it says "Possible match found", answer Yes, this is it or Not a match.
- Use Dismiss as for items that do not matter, and pick a reason.
Request what is missing
Press Request from provider on 1 item, or Request all from provider for a group. The app opens your own email program with a drafted request. It does not send anything for you.
Press Export to download the list as an Excel file. It has a summary sheet and 1 sheet per status.
Look for gaps the list cannot see
The list only finds records that another record mentions. Some gaps leave no trace like that. Check for them by hand.
- Gaps in time. On the Timeline tab, pick Date by date and step through the stay. A missing night of nursing notes shows as a jump in the dates.
- Missing parts of the chart. Press Filters and narrow by document type. Look at each type in turn. If nursing or lab records are thin, ask for more.
- Words that point to other records. Use search in Keywords mode. Try words like "addendum", "see attached", "called" or "pending". See search your records.
- Results with no follow-up. Ask in Q&A which results have no later note about them. Check each cited page.
Mark what you find with a flag or a note, so you can find it again. See add flags, highlights and notes.
What the app cannot find
- It cannot know about a visit or test that no record mentions.
- It does not read EHR audit trails, so it cannot tell you if a note was changed. Request the audit trail on its own. Our guide on EHR audit trails explains how.
- It does not fetch records. You send each request.
When new records arrive, add them to the case. Then see which records to upload for parts that are often left out.
Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
