Set up a medical malpractice case and pick its focus
To set up a med mal case in Medrecords AI, pick Medical Malpractice as the case type and upload the records. After the first read, the app asks you to confirm the case. You check the forum state, the key date, the focus and the body parts. These steer the whole review, so take care.

Pick the case type
When you create the case, pick Medical Malpractice from the case type list. Your role only changes the order of that list. Every role can open every case type.
Use Medical Malpractice for birth injury files too. There is no separate birth injury type. See choose the right case type.
Upload the records
Upload the chart and any other records you have. You can add more later. See which records to upload for a list.
Confirm the case details
When the first read is done, the case status changes to Needs confirming. The Confirm case details window opens on its own. The full steps are in confirm the case details.
For a med mal case, check these items:
| Item | What to check |
|---|---|
| Case summary | A short account of what happened. Fix anything wrong |
| Harm at issue | How the harm happened, as the records describe it |
| Focus | What the case is about, such as a delayed diagnosis. Remove items that are not part of the claim |
| Body parts | The body parts named in the records. Remove or add |
| Date of death / incident | The key date the case turns on, such as the date of the surgery or the missed result |
| Forum state | The state where the matter is filed |
The case details section
The window has a section called Case details. For Medical Malpractice it holds 1 field: Forum state. The forum state is where the matter is filed. That is not always where the care took place.
Nothing in this section is required. A field left empty is recorded as not stated in the records. The app fills in what it can from the records, and you check it.
Pick the focus with care
The focus tells the app what to look for when it reviews every record. A tight focus gives a tighter review.
- Read each focus item the app found. Click its citation to see the source page.
- Remove items that are not part of the claim.
- Type your own item in "+ add your own" if the app missed one.
- Check the key date. If the app found more than 1 date, press Make primary on the right one.
- Press Add date for other dates that matter, such as a return to the emergency room.
You must keep at least 1 focus item, 1 body part or a main key date.
Confirm, then write reports
Press Confirm and continue. Confirming is free. It builds the chronology and the narrative.
You write reports later in the Report studio. For Medical Malpractice, 3 reports start pre-selected: Record Review, Standard of Care Analysis and Causation Analysis. Each new report costs 100 credits and includes 1 verification and cross-examination run. See write the standard of care, causation and merit reports.
Check before you confirm
After you confirm, you cannot edit the focus, the key dates, the body parts or the case details. The 1 exception is when you add records to a finished case. The confirm window opens again then.
Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
