Find prior injuries and check the mechanism across visits

Medrecords AI finds prior injuries in 2 ways. The Pre-existing Condition Analysis report lists earlier care to the same body parts, with cites. The timeline, filters and search let you check it yourself. To check the mechanism, which is how the injury happened, compare each record's account with search and Q&A.

Why these 2 checks matter

The other side will ask 2 things. Was this body part hurt before? Does the story of the injury stay the same from visit to visit? You want to know the answers first, from the records.

Find prior injuries with the report

  1. Open Report Studio and press New report.
  2. Pick the case and the Pre-existing Condition Analysis report type.
  3. Press Generate. A new report costs 100 credits, with 1 verification run.

The report has these sections:

SectionWhat it covers
Records ReviewedThe records used
The Regions and Conditions at IssueThe body parts and conditions in the claim
Documented Pre-incident TreatmentCare before the date of injury
Pre-incident BaselineHow the person was before
Post-incident ChangeWhat changed after
Degenerative and Age-related FindingsWear and tear the records name
Intervening EventsOther events after the injury, such as a new fall
Provider Statements on Apportionment or AggravationWhat doctors wrote about old versus new harm

Apportionment means splitting harm between an old cause and a new 1. Aggravation means the injury made an old problem worse. The report quotes what providers wrote. It does not decide either question.

Find prior injuries by hand

  1. On the Timeline, press Filters. Pick a body part from the claim.
  2. Set a date range that ends the day before the date of injury.
  3. Read what is left. These are the earlier records for that body part.
  4. Make a flag called "Pre-existing" and put it on each page you want to keep. See add flags, highlights and notes.

Then search with Keywords: "prior", "history of", "chronic", "degenerative" and "previous accident". Each keyword gets its own colour.

Ask in the Q&A pane too. Type in the box "Ask about this case": "List all care to the neck before the date of injury, with dates and page cites." Change the body part to fit your case.

Check the mechanism across visits

The mechanism is how the injury happened. Each record may tell it a bit differently. The ER note may say 1 thing and the therapy intake another.

  1. Search with Keywords for words from the account, such as "rear-ended", "slipped", "tripped" or "bitten".
  2. Open each hit and read the account.
  3. In the Q&A pane, type in the box "Ask about this case": "Set out each account of how the injury happened, with the date, the provider and the page."
  4. Add a note where accounts differ.

On an MVA case, Q&A is set up to put differing accounts of the crash side by side. It states the gap between the crash and the first visit, with both dates. It quotes imaging words such as "degenerative" or "no acute finding" exactly. It never treats car damage as proof of injury.

The Mechanism-of-Injury Consistency Analysis

MVA cases can also make this report. Its sections are:

  • The Mechanism as Documented
  • Injuries Claimed and When Each Was First Documented
  • First Care and Early Findings
  • Course and Gaps
  • Prior Conditions and Prior Accidents
  • Imaging Qualifiers and Provider Statements
  • Consistencies and Inconsistencies the Records Show

It reports what the records show. It does not say who was at fault or whether the crash caused the injury.

See prepare a damages summary and a demand narrative for the other reports. Our bodily injury claims page covers how claims teams use these checks.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
