Find gaps in treatment and the reason the records give

Medrecords AI does not flag gaps in treatment for you. You find them with the timeline, filters, search and Q&A, and each fact has a page cite. Start with the missing records list, because a gap is often a record you do not have yet. Then look for the reason the records give for each break.

What a gap in treatment is

A gap in treatment is a break between visits. The other side often says a long break means the person got better, or that something else caused the later pain. So you need to know where each gap is and what the records say about it.

There are 2 kinds of gap:

  • A missing record. The person had care, but the file does not have it.
  • A real break. The person had no care for a while.

Rule out the first before you call it the second.

Step 1: check the missing records list

  1. Open the missing records list from the Needs attention card on the Overview.
  2. Look at the Outstanding tab.
  3. Use the Type menu to show therapy, consult and imaging items.
  4. Request what is missing with Request from provider.

See work through the missing records list. Every case type can also make the Missing Records & Gap Analysis report. It comes with an Excel download of the list.

Step 2: find the breaks

  1. Open the Timeline tab.
  2. Look at the month strip at the top. It shows how many events fall in each month. A month with few or no events can be a gap.
  3. Press Filters and pick 1 provider, such as the physical therapy clinic. Now the space between visits is easy to see.
  4. Switch to Date by date and step through with Next date.

Step 3: ask Q&A

Open the Q&A pane on the case. Type a plain question in the box "Ask about this case". For example: "List each break of more than 30 days between visits after the date of injury. Give the date on each side and any reason the records give." Pick the number of days that fits your case.

Click each cite and read the page. Q&A answers come from the records, but you check them. The Ask a question button at the top of the app is the help chat. It cannot see your cases.

Step 4: find the reason

Records often say why care stopped. Use Keywords search with words like these:

  • "cancelled", "no show" and "missed"
  • "out of town" and "moved"
  • "insurance" and "authorization"
  • "discharged" and "home exercise"
  • "improved" and "worse"

When you find a reason, add a note to the entry. You can also make a flag called "Gap" and put it on the page. See add flags, highlights and notes.
