Upload the right records and bills for a personal injury case

For a personal injury or crash case in Medrecords AI, upload the treating records, the bills and any records from before the injury. Put them all in 1 case for 1 claimant. Then use the missing records list, the provider filter and search to find providers that the records name but you have not requested yet.

EMS and ambulance recordsFirst account of the injury and early findings
ER or urgent care recordsFirst care and the first complaints
Imaging reportsThe radiologist's words, such as "no acute finding"
Specialist and surgery recordsDiagnoses, procedures and work limits
Physical therapy and chiropractic notesThe course of care and any breaks in it
Pharmacy recordsMedicines and refill dates
Itemized billsThe billed charges for each visit
Lien letters and letters of protectionWho claims to be paid back
Records from before the injuryEarlier care to the same body parts
IME report, if there is 1Needed for a Response to IME report

An IME is an independent medical exam, often done by a doctor the other side picks.

Records from before the injury

Ask for records from before the date of injury too. They show the baseline, which is how the person was before. Medrecords AI keeps them in date order with the rest. The Pre-existing Condition Analysis report and Q&A use them to compare before and after. See find prior injuries and check the mechanism.

Bills

Upload itemized bills, not only totals. Billed charges show in reports under "Medical Expenses (Billed Charges)" or "Billed Charges of Record". The Billing Ledger tab lists every charge on the case. Press Export CSV there to save it.

Medrecords AI reports what was billed. It does not say what a bill should be or what is owed after a lien.

File types

  • PDF, PNG, JPG, Word and text files are read. ZIP files are unpacked in your browser.
  • TIFF, GIF and HEIC files are not read. Save them as PDF or JPG first.
  • DICOM imaging files are stored and you can view them. The app does not read the images. Upload the radiology report too.

Duplicate pages

Injury files repeat pages a lot. The same ER record can come from the hospital, the doctor and the insurer. Keep Remove duplicate pages on. Credits for removed copies come back to you. Our guide on deduplicating personal injury records explains why this matters for bills.

Find providers you have not requested

Records often name other providers. A referral to a pain clinic or an order for an MRI are common examples. Here are 4 ways to find them.

  1. Missing records list. It lists records your documents mention but the file lacks. Items are grouped by provider. See work through the missing records list.
  2. Provider filter. On the Records tab, open the filters and look at the Provider and Facility lists. Any name you do not know is worth a look.
  3. Search. Use Keywords and try "referred", "referral" and "follow up with".
  4. Q&A. Open the Q&A pane and type in the box "Ask about this case": "List every provider and facility named in the records, with the first page that names each." Click each cite to check it.

The missing records list only finds records that another record mentions. If nothing in the file points to a visit, the app cannot know it happened. Ask the claimant too.

Add records later

New records often come in after you start. Add them to the same case. Do not make a new case. See add records to an existing case.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
