Upload the fact sheet as a record, then find missing providers

Medrecords AI has no plaintiff fact sheet feature. You can still use the fact sheet to find gaps. Upload it into the claimant's case as a record. Then ask Q&A which providers it names that have no records in the file. Check the missing records list too, and request what is still outstanding.

Why this takes a few steps

A plaintiff fact sheet is the form a claimant fills in for the court. It lists doctors, pharmacies and hospitals. The app does not read it as a form or check it field by field. It reads it like any other page. So the work of comparing it to the file is yours, with the app's help.

Upload the fact sheet

  1. Open the claimant's case.
  2. On the Files card, press Add files.
  3. Add the fact sheet as a PDF.
  4. Press Upload.

You pay 1 credit per page, as for any record. The confirm window opens again after the upload. Your earlier answers are kept. See add records to an existing case.

Ask which providers have no records

Open the Q&A pane on the case. Type in the box that says "Ask about this case". Try questions like these:

  • "List every provider, pharmacy and hospital named in the fact sheet."
  • "Which of those providers have no records in this file?"
  • "Which records cover the exposure period, and which months have none?"

Each answer carries numbered citations. Click a citation to open the page. Check each name against the Provider and Facility filters on the Records tab. See filter and group the documents index.

Use the missing records list

The missing records list shows records that your documents mention but that are not in the file. A lab result that was ordered but never arrived is a common example.

  1. Open the list from the Needs attention card on the Overview.
  2. Use the Type menu or Search references to narrow it.
  3. Answer Yes, this is it or Not a match on each possible match.
  4. Use Dismiss as for items you do not need. Nothing is deleted.

The list only finds records that another record mentions. If no record refers to a provider, the list cannot know about it. That is why the fact sheet check above matters. See work through the missing records list.

Request the records

  • Press Request from provider on 1 item, or Request all from provider for a group.
  • The app opens your own email program with a drafted request. It does not send anything for you.
  • Press Export to download the list as an Excel file. This costs no credits.

For providers you found through Q&A but that are not on the list, write the request yourself.

Keep the case up to date

When new records arrive, add them to the same case. Then check the list again. Items the app matches move to the Located tab.

For a written gap summary, you can also make a Missing Records & Gap Analysis report. See which report to write.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
