Work through a fictional HIE case from upload to expert packet

This is a made-up birth injury case, worked from start to finish in Medrecords AI. The people, the hospital and the facts are invented. It shows how a nurse consultant sets up the file as Medical Malpractice, finds the labor facts with page cites, chases missing records and builds a cited packet for the expert.

The case

Maya gave birth to her son, Leo, at the invented Riverbend General Hospital on March 3, 2024. Labor was long. Pitocin was used. Leo was born by C-section and went to the NICU. He was later found to have HIE, a brain injury from too little oxygen around birth.

Grace is the nurse consultant on the file. She has her own account. The case belongs to her.

Day 1: set up the case

  1. Grace starts a new case and picks Medical Malpractice.
  2. She puts the files in 2 folders, "Mother" and "Baby", and zips them. The folder names carry into the file names.
  3. She leaves Remove duplicate pages on and uploads.

When the case shows Needs confirming, she opens Confirm case details. She sets the Forum state. She makes March 3, 2024 the main key date with Make primary. With Add date she adds March 2, 2024 for the start of labor and March 5, 2024 for the brain MRI, with a note on each. She types "brain injury at birth" in "+ add your own". She presses Confirm and continue.

See set up a birth injury case.

Day 2: find the gaps

The missing records list opens on its own. On the Outstanding tab, Grace sees a cord gas that a note says was sent, with no result on file. She also sees an EEG named in the NICU notes, with no report.

The list does not check strips, so she checks by hand. On the Timeline she steps through March 2 and March 3, 2024 with Date by date. The nursing notes describe the tracing from 9:00 pm to 4:15 am. The strips on file stop at 1:30 am. She flags the last strip page and adds a note: "Strips after 1:30 am not produced."

She presses Request all from provider for Riverbend General Hospital. Her email program opens with a draft. She adds the missing strip hours by hand and sends it herself.

Day 3: list the labor facts

Grace searches in Exact mode for "decel" and in Keywords mode for "Pitocin" and "Apgar". Then she opens the Q&A pane and types each question in the box that says "Ask about this case":

  1. List every Pitocin start, rate change and stop, with the time and the page.
  2. When did staff call the doctor during labor? Quote each note.
  3. What were Leo's Apgar scores, and at what minutes?

Each answer has page cites. She clicks each one and checks the time and value on the page. Here is what her notes say:

TimeFact as chartedChecked on page
March 2, 2024, 8:00 pmPitocin startedYes
March 3, 2024, 12:40 amPitocin rate raisedYes
March 3, 2024, 1:10 amNurse notes late decelerationsYes
March 3, 2024, 2:55 amDoctor called, per nursing noteYes
March 3, 2024, 3:50 amDecision for C-section chartedYes
March 3, 2024, 4:30 amLeo delivered. Apgars 2 and 5Yes

She notes 1 thing the app cannot do: it did not read the strip tracing. That stays with the expert.

See find Pitocin changes, decelerations and Apgars.

Day 10: add the new records

The cord gas result and the late strips arrive. Grace adds them to the same case. The confirm window opens again with "new" items. She checks them and confirms.

Day 11: write the reports

In the Report studio, Grace writes the Medical Malpractice Record Review, the Standard of Care Analysis and the Causation Analysis. Each new report costs 100 credits, with 1 verification run. She runs Run Verification and Cross-Examination on each and opens every cited page for times and scores.

The expert packet

Grace sends the expert:

  • The 3 reports, downloaded as PDF
  • The chronology PDF with her notes beside the entries
  • Her table of labor facts with page numbers
  • The strips, flagged, for the expert to read
  • The missing records Excel file, showing what is still outstanding

The expert gives the opinion. Medrecords AI does not.

Read our guide on HIE and birth injury medical records for more on these files.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
