Set up a birth injury case with the mother's and baby's records
To set up a birth injury file in Medrecords AI, create 1 case and pick Medical Malpractice as the case type. Upload the mother's records and the baby's records into that same case. Name each file so you can tell whose it is. Then check the key dates and focus when the app asks you to confirm.

Why 1 case for 2 patients
The story of a birth injury runs across 2 charts. The mother's chart shows labor, drugs and the delivery. The baby's chart shows the newborn's condition and the NICU stay. You need both on 1 timeline to see what happened hour by hour.
There is no birth injury case type. There is also no field that marks a record as the mother's or the baby's. Your file names and flags do that job, as shown below.
Create the case
- Start a new case.
- Pick Medical Malpractice as the case type. See choose the right case type.
- Add the mother's files and the baby's files together. ZIP files are fine. They are opened in your browser first.
- Leave Remove duplicate pages on. Hospital productions often repeat pages.
- Check the page count and the cost, then upload.
Name files so you can tell them apart
The file name shows as the source file on the timeline. So put the patient first in each name. For example:
- "Mother - Hospital - Labor and delivery.pdf"
- "Mother - Prenatal clinic - 2024.pdf"
- "Baby - Hospital - NICU.pdf"
- "Baby - Pediatrician - 2024.pdf"
If you upload a ZIP, the folder names go into the file name. A folder called "Mother" with a file inside shows as "Mother - " and then the file name. So you can sort files into 2 folders before you zip them.
On the Timeline, use Group by source file to see the 2 charts apart.
Make 2 flags
Flags help you filter later.
- Open Settings, then Stages and flags.
- Under Record flags, press New flag.
- Make 1 flag called "Mother" and 1 called "Baby". Pick a Colour for each.
- On the Records tab, press the flag button on each record row and pick the right flag.
See add flags, highlights and notes.
Confirm the case details
After the first read, the case shows Needs confirming and the Confirm case details window opens. Medical Malpractice has no extra fields beyond Forum state. Take care with these parts:
| Part | What to do for a birth injury file |
|---|---|
| Focus | Remove items that are not part of the claim. Type your own in "+ add your own", such as "brain injury at birth" |
| Body parts | Remove or add, such as the brain |
| Date of death / incident | This is the key date on Medical Malpractice. Make the delivery date the main date with Make primary |
| More dates | Use Add date for other dates, such as the start of labor or the NICU transfer. Add a note to each |
Press Confirm and continue when you are done. After you confirm, you cannot edit the focus, key dates or body parts. See confirm the case details.
Records that come later
Birth injury records often arrive in parts. Add later batches to the same case, not a new one. See add records later.
Next, read which records to upload, and in what order. Our birth injury record review page has more background.
Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
