Check a birth injury file for missing strips, MAR and NICU pages
Medrecords AI lists records that your file mentions but does not have, such as a lab result that was ordered and never arrived. It does not check fetal monitor strips for missing hours. For strips, MAR pages and NICU notes, you check the gaps yourself with the timeline, filters and search. This page shows how.

Step 1: read the missing records list
The list opens on its own the first time a finished case has missing items. You can also open it from the Needs attention card on the Overview.
- Start on the Outstanding tab.
- Use the Type menu to look at lab, imaging, consult and hospitalization items first. Cord gases, brain imaging and NICU transfers fall in these groups.
- Pick Ordered to see items that were ordered with no result on file.
- For each "Possible match found", answer Yes, this is it or Not a match.
- Use Dismiss as for items you do not need. Nothing is deleted.
The list costs no credits. See work through the missing records list.
Step 2: check the strips by hand
The app has no fetal monitor strip logic. A missing hour of strip only shows on the list if another record mentions it. So check by hand.
- Open the Timeline and press Filters.
- Set the dates to the labor and delivery window.
- Pick Date by date and step through with Next date.
- Use search in Exact mode for "strip" and "EFM". Exact finds parts of words too.
- Write down the first and last time on the strips you have. Compare them with the times in the nursing notes.
If the nurses chart times that the strips do not cover, part of the strip may be missing. Flag the pages and add a note.
Step 3: check the MAR
The MAR shows when each drug was given. Gaps here matter for Pitocin, magnesium and pain drugs.
- Search in Keywords mode for "Pitocin" and "oxytocin". Choose Any so you see pages with either word.
- Open each order and look for a matching MAR entry.
- If an order has no MAR page, note it.
Step 4: check the NICU stay
- Filter by Facility to the NICU or the hospital.
- Look for the NICU admission note, daily notes and a discharge summary.
- Search for "transfer". A transfer to another hospital means a second chart to request.
- Search for "MRI" and "EEG". If the notes mention a study but there is no report, request it.
Things to look for
| Record | A sign it may be missing |
|---|---|
| Fetal monitor strips | Nursing notes describe the tracing at times no strip covers |
| Mother's MAR | An order for Pitocin with no matching dose entries |
| Cord gas | A note says a gas was sent, but no result page is on file |
| Delivery room notes | The baby's chart starts at the NICU, not at birth |
| NICU records | The discharge summary names a study with no report |
| Transfer records | A note says "transferred" with no chart from the other hospital |
Request what is missing
Press Request from provider on an item, or Request all from provider for a group. The app opens your own email program with a drafted request. It does not send anything for you. Press Export for an Excel file of the list.
For a written report on the gaps, use Missing Records & Gap Analysis. See pick your reports. When the records arrive, add them to the same case.
Our page on birth injury record review lists the records most files need.
Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
