Which labor, delivery and NICU records should you upload?

For a birth injury file in Medrecords AI, upload the mother's prenatal, labor and delivery records and the baby's newborn, NICU and follow-up records. Put them all in 1 Medical Malpractice case. Order does not change how the app reads them, but a clear order and clear file names make your own review faster.

The mother's records

  • Prenatal clinic notes and ultrasound reports.
  • Hospital admission notes for labor.
  • Labor and delivery nursing notes and flow sheets.
  • Fetal monitor strips, if the hospital produced them.
  • Physician and midwife progress notes.
  • Medication administration record (MAR), including Pitocin and pain drugs.
  • Orders, including any order to start, raise or stop Pitocin.
  • Anesthesia records for an epidural or a C-section.
  • Operative report, if there was a C-section or an assisted birth.
  • Delivery summary and discharge summary.
  • Billing records, if you need the charges.

The baby's records

  • Newborn delivery room notes and Apgar scores.
  • Cord blood gas results and other early lab work.
  • Neonatal resuscitation notes.
  • NICU admission note, progress notes and discharge summary.
  • NICU MAR and orders.
  • Brain imaging reports, such as MRI and head ultrasound.
  • EEG reports, which record brain waves.
  • Transfer records, if the baby moved to another hospital.
  • Later pediatric, neurology and therapy records.

A simple upload order

The app sorts records by date, oldest first. So the order you upload in does not change the timeline. Still, it helps to work in this order so nothing gets missed:

  1. The mother's prenatal records.
  2. The mother's hospital stay for the birth.
  3. The baby's birth and NICU stay.
  4. The baby's later care.
  5. Billing records last.

Name each file with the patient first, such as "Mother - Hospital - Labor.pdf" or "Baby - Hospital - NICU.pdf". See set up the case for why.

File types to watch

What you haveWhat to do
PDF or Word filesUpload as they are
PNG or JPG scansUpload as they are
TIFF, GIF or HEIC filesSave them as PDF or JPG first. The app does not read these
DICOM imaging, such as the baby's MRIUpload it. It is stored and you can view it, but the app does not read the images. Upload the written report too
A ZIP from the hospitalUpload it. It is opened in your browser first

Fetal monitor strips are a special case. The app has no strip logic and does not read the tracing line. Any printed text on a strip page is read like other text, but faint scans give weaker text. Keep the strips in the file so you can view them and flag pages for your expert.

When records come in parts

Hospitals often send the L&D chart, the strips and the NICU chart at different times. Add each new batch to the same case. Leave Remove duplicate pages on. Pages you already have are taken out before they are read. See add records later.

Once the records are in, check the file for missing records. Our guide on HIE and birth injury medical records explains these records in more detail.

Medrecords AI organizes and cites the records. Its output is not a medical or legal opinion and does not replace your expert.
